Employee Forms
The paperwork our clients need most often, ready to download. If you are not sure which form applies, call your rep at 504.520.8368 and ask.
Hiring someone
Form I-9, Employment Eligibility Verification
Confirms a new employee is authorized to work in the United States. Every employer has to collect one for every hire.
Form W-4, Employee's Withholding Certificate
Sets how much federal income tax comes out of each paycheck. New employees complete it before their first payroll run.
Form L-4, Louisiana Withholding Exemption Certificate
The Louisiana version of the W-4. It sets state income tax withholding for employees working in Louisiana. This is the current R-1300 (1/26) revision.
Direct Deposit Authorization
Authorizes us to deposit an employee's pay into their bank account. Needed before their first direct deposit, and again any time they change banks.
New Hire Packet (Louisiana)
Our employee data sheet together with the Louisiana L-4 and the 2026 W-4, in one document. The data sheet also carries the direct deposit authorization. For employees outside Louisiana, use the individual forms above instead.
Paying a contractor
Form W-9, Request for Taxpayer Identification Number
For independent contractors and vendors rather than employees. Collect it before the first payment so year-end 1099s are correct.
When someone leaves
Louisiana Separation Notice
Louisiana requires employers to give this to an employee who leaves, whatever the reason for the separation.
